Patient consent can be supported in several ways. Choose the option—or combination of options—that best suits your practice’s workflow.

We recommend making patients aware of Medow before their consultation and giving them an opportunity to ask questions or withdraw consent at any time.

1. Display the patient disclaimer sign

Place the disclaimer sign in your consulting rooms or at reception to help inform patients that Medow may be used during their consultation.

If you did not receive a sign in your welcome box, or would like another copy:

You can also download and print the disclaimer sign here:

2. Use the Medow Patient Informed Consent Form

Practices that prefer to capture consent using a separate form can use the Medow Patient Informed Consent Form below.

3. Add consent to your new patient intake form

If your practice already uses a new patient intake form, you can add the following consent statement:

☐ I consent to my doctor using Medow Health, an Australian-built AI documentation assistant, to help document my consultation. I can withdraw this consent at any time.

4. Share the patient information flyer

Download and print this flyer to help patients understand how Medow is used during their visit, how their information is protected, and the choices available to them.
